Study Consent Form
In the process of creating a psychological study at universities, many steps are taken to ensure that studies meet very strict ethical requirements. At the University of Minnesota, Carolyn Parish built her online study with full cooperation of the Internal Review Board (IRB). The IRB is composed of non-psychology professors who's job it is to make sure that both the study's research topics as well as the study's methods are well within even the strictest definitions of Ethical. The first part of the survey is the consent form that verifies that the respondent knows exactly what they are getting into before they begin the study. I have taken the liberty of modifying my sisters Consent form that took several months of reviews before being approved, into a form that is mildly entertaining as well as mildly instructional. I do not intend any disrespect to the psychology community or it's methodologies with this form or any part of this demo:).

For the purposes of this demo, I have heavily modified my sisters to be humorous for the entertainment of non-psychology related participants. Be sure to note the following:

You are invited to be in a research study of oddity and entertainment. You were selected as a possible participant because of your interest in how my sisters online psychology research at the University of Minnesota works. We ask that you read this form carefully and ask any questions you may have before agreeing to be in the study. This study is being conducted by PersonalityScience.org.
Background Information: The purpose of this study is to investigate exactly what kind of people want to know how this application works. Well, that and what size shoes republicans wear.
Procedures: If you agree to be in this study, you will be asked questions about your political preferences, your favorite color, what soda you drink, and anything else I could think of. You will also be asked some general questions your personality. The length of time needed to complete this study is approximately five minutes.
Risks and Benefits of Being in the Study: There is a large chance that you may feel uncomfortable answering questions we thought up. However, these questions are unlikely to cause any more discomfort than a person would typically encounter in everyday social or educational situations. The benefits of participation are that you learn how exactly this great website works.
Confidentiality and Security: Hey, I'm telling you up front... Everybody and their brother are going to see your individual results. Any data submitted to the site will be seen by every other person taking this tour. All files containing any information you give would have been protected using current Internet security technology provided by the Lotus Domino program, but I chose to turn most of it off for the tour. If you sign up with your email address, we won't associate that with any of the data though. That goes in a separate location that only I can see, though I doubt that will make you feel much better.
Voluntary Nature of the Study: Your decision whether or not to participate will not affect your current or future relations with me. If you decide to participate, you are free to withdraw at any time but you won't get to see the rest of the site. If you do feel uncomfortable answering these questions, I suggest just plain lying about it. I don't care that much about data integrity on this survey anyways. They're pretty stupid questions though, so I figure hardly any of them will make you uncomfortable. Well, maybe those income and underwear questions. If it's all the same to you, I'd appreciate it if you told the truth.
